Job Description

We are seeking a Care Manager (RN) to join our dynamic team. The ideal candidate will have strong experience in care management, utilization review, and care coordination and a proven ability to improve patient outcomes through evidence-based plans of care.

Responsibilities:

• Assess member needs and develop individualized care plans.
• Coordinate services across providers and care settings.
• Conduct utilization review and ensure appropriate level of care.
• Educate members and families on conditions, treatments, and resources.

Required Skills & Qualifications:

• Active RN license in good standing.
• Experience with care management or utilization management.
• Effective communication, documentation, and collaboration abilities.
• 3+ years of clinical experience in acute, ambulatory, or managed care settings.

Preferred Skills:

• Certification in Case Management (CCM, ACM).
• Experience with EHRs and managed care platforms.
